Welcome to Altum Care! Our team works both individually and collectively to provide support and guidance. Our Case Managers assist residents with coordinating medical, mental health, dental, vision and podiatry care. Our Residential Support staff offers daily instruction to assist Residents in being able to live with a minimal amount of assistance. Our Licensed and Certified Rehabilitation Counselors are available and always on call to provide counseling support. PHH is a licensed, specialized 24-hour, seven days a week operation.
